Requirements for Applying for a Visa
Requirements for applying for a visa at the consulate are as follows:
- Fill out an application form and submit it with one passport size photo
- Submit copy of old passport or document that shows your date of birth
- Submit copy of marriage certificate if you are married
- Submit copy of birth certificate if you have children under 18 years old
- Submit proof that you have sufficient funds to cover the cost of living during your stay in the Philippines
Passport Renewal
If you are applying for a new passport, you will need to provide your old passport and two (2) photocopies of it. You also need to bring your original birth certificate as well as two (2) photocopies of it. In addition, if you have been naturalized as a US citizen, bring your naturalization certificate and two (2) photocopies of it.
If you are renewing and existing US passport, then bring the same documents that were required when you applied for a new one.

Who can Go to the Consulate and What Documents do They Need to Bring with Them?
Any Filipino citizen or resident, regardless of status, who needs consular services may contact the Philippine Consulate Chicago. This includes overseas Filipinos (OF’s), returning OF’s (Ross), and Odds (Overseas Filipinos Departing).
What documents do they need to bring with them?
All applicants for consular services must submit their valid passports with at least six months validity from date of application plus one photocopy each. If applicant has dual citizenship, he/she will be required to present his/her documents.
