How do I apply for a visa at the Consulate?

To apply for a visa at the embassy, please come in person with the following:

  1. Valid Passport
  2. One (1) original passport-sized photo of yourself taken within the last six months
  3. Proof of residence in the Dominican Republic – such as a copy of your lease or utility bill
  4. Completed application form
  5. Payment of consular fee
